Payment Policy
For contracted health plans, such
as HMOs, PPOs, and Medicare, we
collect co-pays, co-insurance,
and/or deductibles at the time of
service.

you can leave a message with our
toll-free number, 24 hours a day,
at 888.557.5501.
If you have a medical emergency,
call 911, or go to the nearest
emergency room for immediate
assistance.
